How to make a referral

If you would like to refer one of your patients please contact us.

You can either register and complete the form yourself or your practice administration staff can register and complete the form on your behalf. There is also the option to scan in letters / records without having to complete the form from scratch. The pages are encrypted, safeguarding confidential patient information, and does not send anything by email.

GP Education

I am very keen to liaise with GPs to help with education on the treatment of upper limb (shoulder, elbow, wrist and hand) conditions in the primary care setting, this includes examination as well as injections that can be performed in primary care.
